Your Care Manager provides guidance navigating housing resources — helping you apply for affordable housing, understand Section 8, connect with homeless prevention programs, find transitional housing, access rental assistance, and coordinate with housing navigators in your county. We work in Los Angeles, Orange, San Bernardino, and Riverside counties.
We use closed-loop referrals, which means we do not simply hand you a list of numbers. Your Care Manager connects you to the right program, follows up with the partner and with you, and keeps working the plan until housing support is actually in place.
