Food insecurity and chronic illness are deeply connected. Your Care Manager listens to your situation and connects you with the right food resources — medically tailored meal programs designed for conditions like diabetes, heart disease, and kidney disease, plus food banks, community pantries, CalFresh/EBT enrollment support, and nutrition counseling referrals.
Because we use closed-loop referrals, we do not simply hand you a phone number and walk away. Your Care Manager follows up with you — and with the partner program — to confirm that the food support actually reached you and is meeting your needs.
