Find out if you or a loved one qualifies for Enhanced Care Management (ECM). We work exclusively with Medi-Cal members enrolled in Anthem Blue Cross or Kaiser Permanente across Los Angeles, Orange, San Bernardino, and Riverside counties.
ECM participation is voluntary. You may opt out of Enhanced Care Management at any time by contacting your Care Manager or your Medi-Cal health plan. ECM services are available to eligible Medi-Cal Managed Care members and are subject to health plan authorization and Population of Focus criteria.

Dignity ADHC ECM is contracted to serve adult populations of focus. Eligibility is based on the specific populations of focus covered by your Medi-Cal health plan.
RequiredYou must be an active Medi-Cal member enrolled with Anthem Blue Cross or Kaiser Permanente. We do not serve members of other plans.
RequiredWe serve Los Angeles, Orange, San Bernardino, and Riverside counties. You must reside in one of these four counties.

RequiredYou must be an active Medi-Cal member. ECM is a covered benefit of your Medi-Cal managed care plan.
RequiredMembers can be identified by their health plan, self-refer, or be referred by a provider. Dignity ADHC ECM may assist with screening and referral; your health plan determines final ECM eligibility and authorization.
Varies by PlanThe Department of Health Care Services (DHCS) defines these populations for ECM. Each health plan confirms eligibility based on the plan's specific populations of focus.
Severe mental illness (SMI), substance use disorder (SUD), at risk of long-term care institutionalization, and high utilizers of care.
Individuals and families experiencing homelessness, individuals at risk for avoidable hospital or ED utilization, individuals with serious mental health and/or SUD needs, adults at risk for long-term care institutionalization, adult nursing facility residents transitioning to the community, and birth equity (pregnant and postpartum individuals).
