You must be 18 years of age or older to enroll in Enhanced Care Management with us.
RequiredYou must be an active Medi-Cal member enrolled with Anthem Blue Cross or Kaiser Permanente. We do not serve members of other plans.
RequiredWe serve Los Angeles, Orange, San Bernardino, and Riverside counties. You must reside in one of these four counties.
RequiredECM is for Medi-Cal members with complex health and social needs. Your plan identifies ECM populations of focus — see the list below.
RequiredYou must be an active Medi-Cal member. ECM is a covered benefit of your Medi-Cal managed care plan.
RequiredMembers can be identified by their health plan, self-refer, or be referred by a provider. We'll work with your plan to confirm eligibility.
Varies by PlanThe Department of Health Care Services (DHCS) defines these populations for ECM. Each health plan confirms eligibility based on the plan's specific populations of focus.
Severe mental illness (SMI), substance use disorder (SUD), at risk of long-term care institutionalization, high utilizers, and individuals/families experiencing homelessness.
Individuals/families experiencing homelessness, birth equity, pregnancy/postpartum, high utilizers, and members with SMI.
