What Are Community Resource Connections?

Dignity ADHC ECM does not provide crisis intervention, a hotline, or suicide prevention services directly. Our Care Managers connect you to community and social support services that address the everyday factors impacting your health — such as housing, food, utilities, transportation, and behavioral health — through licensed community partners and public resources.

We use closed-loop referrals, which means your Care Manager confirms that each referral actually took place and that you received the services you were referred to. If you are experiencing a mental health crisis, your Care Manager will also connect you to the national 988 Suicide & Crisis Lifeline or your county crisis team, which are delivered by licensed community partners.

What does medical visit support include?
Your Care Manager coordinates care across your providers, helps with referrals between specialists, supports appointment scheduling, follows up with providers after visits, translates medical information into clear action items, and advocates for you during transitions of care.
Do you attend appointments with me?
No, this is not companion support. Your Care Manager does not attend appointments with you. Instead, we coordinate scheduling, communicate with your care team, and follow up afterward to make sure the plan is clear and next steps happen.
Can my Care Manager contact my doctor?
Yes. With your consent, your Care Manager can communicate with your providers and health plan to coordinate referrals, appointments, and follow-up on your behalf.
How often will I meet with my Care Manager?
It depends on your care plan. Some members connect with their Care Manager weekly, others every few weeks. Check-ins can happen by phone, in person, or telehealth, based on what works for you.
Does Dignity ADHC ECM drive me to my appointments directly?
No. We don't operate vehicles and we don't provide companion support. Your Care Manager helps you access the non-emergency medical transportation benefit that your Medi-Cal managed care plan offers.
Which Medi-Cal plans can I use this with?
We work with Anthem Blue Cross and Kaiser Permanente Medi-Cal members who are enrolled in Enhanced Care Management (ECM).
What types of appointments qualify for transportation?
The benefit is designed for medical appointments, including primary care, specialist visits, and ongoing treatment. Your Care Manager can confirm what your specific plan covers.
How soon should I tell my Care Manager about an upcoming appointment?
Let your Care Manager know as soon as the appointment is scheduled. Setting up transportation ahead of time helps make sure everything is in place when you need it.
What documents do I need to bring?
Typically you will need a valid ID, proof of income, proof of residency, and Social Security information. When you schedule your appointment, we will provide a complete checklist based on the specific benefits you are applying for.
How long does it take to get approved for benefits?
Processing times vary by program. CalFresh applications are typically processed within 30 days, while Medi-Cal may take 45 days. SSI applications can take 3-6 months. We will keep you updated on the status of your applications throughout the process.
