Your Care Manager provides closed-loop referrals to community and social support services that address social determinants of health — housing, food, behavioral health, transportation, utilities, and more. If you are in crisis, please call or text 988 for the Suicide & Crisis Lifeline.
Dignity ADHC ECM does not provide crisis intervention, a hotline, or suicide prevention services directly. Our Care Managers connect you to community and social support services that address the everyday factors impacting your health — such as housing, food, utilities, transportation, and behavioral health — through licensed community partners and public resources.
We use closed-loop referrals, which means your Care Manager confirms that each referral actually took place and that you received the services you were referred to. If you are experiencing a mental health crisis, your Care Manager will also connect you to the national 988 Suicide & Crisis Lifeline or your county crisis team, which are delivered by licensed community partners.
