We coordinate and connect; housing availability and placement are determined by the programs and agencies we refer you to.
Your Care Manager provides guidance navigating housing resources — helping you apply for affordable housing, understand Section 8, connect with homeless prevention programs, find transitional housing, access rental assistance, and coordinate with housing navigators in your county. We work in Los Angeles, Orange, San Bernardino, and Riverside counties.
We use closed-loop referrals, which means we do not simply hand you a list of numbers. Your Care Manager connects you to the right program, follows up with the partner and with you, and continues advocating as you work through the process. Please note that housing availability and placement decisions are made by the housing programs and agencies we refer you to — we cannot guarantee housing placement.
